How it works
Molina members can earn rewards by completing approved healthy activities, then redeem them for gift cards.
Log in to My Molina
Access the member portal or app and go to “My Wellness” > “Healthy Rewards.”
Complete the Health Assessment
Answer a health questionnaire to begin the enrollment process.
Visit & Unlock Rewards
Once verified, your visit triggers your earned reward.
How to Qualify for Healthy Rewards
- You hold a Molina plan at the time of service.
- Services are performed during the reward year and submitted by the deadline (e.g. for 2025, before January 31, 2026 in AZ).
- Molina verifies your service claims before issuing your reward.
- For some programs, you must submit a reward claim form after the service.
- Eligibility, reward types, and amounts change depending on your state.
Molina Healthy Rewards You Can Earn
Depending on your plan and state, you may earn:
- Gift cards ($200 gift card for completing assessment + wellness visit)
- Specialty rewards for children ($50 reloadable reward card for Well-Care visits ages 3-21)
- Cancer screening rewards ($100 reloadable reward card for Breast Cancer Screening – Mammogram Ages 40-74)

- Gift cards and reloadable reward cards are available to qualifying Molina members.
- You must have Molina Healthcare as your primary insurance at the time of service to earn a reward.
- After you complete your annual wellness visit and health questions, Molina will email you instructions on how to claim your gift card.
- Please allow 6–8 weeks after Molina receives and confirms your claim for processing.
- For child and cancer screening rewards, a reloadable reward card is mailed with additional details within 2–4 weeks.
- Rewards are only issued once services are confirmed and submitted within the valid reward period.
- Rewards, eligible activities, and reward amounts may vary by state and by specific Molina plan.
- Some rewards may only apply in select markets or for certain member types. Always refer to your plan materials or member portal to see which rewards are available to you.
