How it works
UnitedHealthcare members can earn rewards by reaching program goals (ongoing and daily behaviors) and by completing one-time reward activities.

Open the app, select UHC Rewards, and activate the program to begin earning.

How to Qualify for UHC Rewards
You may be eligible to participate in UHC Rewards if:
- You’re enrolled in an eligible UnitedHealthcare health plan
- You are 18 or older
- Your plan includes UHC Rewards at no extra cost
- You download the UnitedHealthcare app or access myuhc.com
- You activate UHC Rewards inside the app to begin earning
- You complete approved healthy activities like surveys, checkups, or daily goals
- You connect a fitness tracker to unlock additional earning opportunities (Optional but recommended)
UHC Rewards You Can Earn
Examples of reward amounts in some plans:
- $25 for completing a health survey
- $50 for a preventive checkup
- $5.25 per week for meeting step goals
- $10 for 14 nights of sleep tracking
- $5 for switching to paperless

We understand that choosing a health insurance plan that meets your needs can be stressful. Speak to a licensed Coverall health insurance representative that will make it easy and stress free to choose a plan that’s right for you.
- Gift cards and reloadable reward cards are available to qualifying Molina members.
- You must have Molina Healthcare as your primary insurance at the time of service to earn a reward.
- After you complete your annual wellness visit and health questions, Molina will email you instructions on how to claim your gift card.
- Please allow 6–8 weeks after Molina receives and confirms your claim for processing.
- For child and cancer screening rewards, a reloadable reward card is mailed with additional details within 2–4 weeks.
- Rewards are only issued once services are confirmed and submitted within the valid reward period.
- Rewards, eligible activities, and reward amounts may vary by state and by specific Molina plan.
- Some rewards may only apply in select markets or for certain member types. Always refer to your plan materials or member portal to see which rewards are available to you.
